Aftercare Planning
The goal of the Aftercare Program is to support the child and their family through the transition and reintegration from out of home placement to community living.
Objective:
• Customized program to meet the child, family, and agency needs
• Develop, support, monitor, and implement the Aftercare Plan using a community support model
• Balance length of stay with individualized Aftercare Services
Research and outcome studies clearly illustrate that residential facilities with formal Reintegration and Aftercare Programs are able to decrease length of stay and enhance treatment outcomes. Clinicare facilities offer a client centered transitional reintegration plan with a 90-day Aftercare Program that encompasses the community support model. Each Academy employs or works with a local Reintegration-Transition coordinator who assists the family and agency in returning that child to the community and linking them with a support network. Changes made in that treatment milieu are best kept in place by the existence of social networks that affirm the changes through support, vigilance and consistency. Each program is individually developed in cooperation with the family and agency.
In activating the natural support network the Transitional Reintegration Coordinator provides the following components:
• Facilitates school re-enrollment
• Resource guide to agency and family
• Conduit to employment
• Ensure medication management
• Facilitate referral to psychiatrist
• Provide and coordinate mental health services
• Urinalysis enforcement
• Weekly communication with parents and agency
• Provide or coordinate family therapy
• Weekly monitoring and face to face visits
• Attendance monitoring
• Provide training to parents on behavioral modification programs
• Provide outcome and progress reports to agencies
