Admissions

The Wyalusing Academy is a coed facility licensed to accept children between the ages of 10 and 18 years with:

• Emotional Disorders
• Conduct Disorders
• Behavior Disorders
• Mild Mental Retardation/Neurological Impairment
• Aggressive Behavior
• Impaired Social Relationships
• Delinquent Activity
• Sexualized Behaviors
No restrictions based upon religion, race or sex.

Admission Process:
Intake phone calls and/or referral letters are initially screened by the Director of Admissions. The Director of Admissions requests that the following evaluation materials be included in the actual referral packet:

• Psychiatric Evaluation
• Psychological Evaluation
• Social History
• Academic Information
• Placement History
• Medication History (if applicable)

The Director of Admissions reviews the referral material and consults with the Medical Director, Director of Social Services and other professional staff as needed. The Director of Admissions will contact the referring agency to inform them of the placement decision and if accepted, a projected placement date will be established.

Referrals that are affirmed appropriate by the Director of Admissions precipitates sending an admission package of materials including:

• Parental Consent Forms
• Interagency Agreement
• Parental Handbook
• Resident Handbook
• Suggested Clothing List

A copy of the birth certificate, court order, physical completed within the last year, and permanency plan are requested. The completed admission packet and medical assistance card are required prior to admission.

Placements by out-of-state agencies must secure Interstate Compact approval prior to a placement date being established.
