Alternative Day School Program
Our mission is to provide a highly structured, supportive and safe learning environment where students, ages 6-19, with unique or special education needs can benefit from education and experience positive growth and success.

Assessment & Evaluations:
Assessment is an ongoing and integral component relating to each student's program plan and progress. Clinicare provides assessment information and teacher evaluations throughout the student's placement.
Assessments include:
IEP and program development at the time of placement
Achievement testing at the time of placement and 12 month intervals.
Initial staffing 2-4 weeks following placement
Daily Home-School Communication Progress Logs
Weekly progress reports to parents and school district
Collaboration:
Through ongoing collaboration we can provide appropriate educational opportunities and services for students. Collaboration involves:
• Students

• School District Team
• Community Agencies
• Clinicare Professionals
Referral Steps:
Step 1: Review of student's school records
Step 2: Student, parent, School District visit
Step 3: IEP Team Meeting
Referrals may be initiated throughout the school year and summer vacation.
Monthly team staffings include:
Academic achievement
Social, emotional, and behavioral considerations
Progress toward annual goals

New Frontier Academy - Prairie du Chien, Wisconsin
Crossroads Academy - Decorah, Iowa
Daily program: 9 a.m. - 3 p.m.

• Develop individualized academic instruction designed to meet each student's needs.
• Assist students in strengthening their problem solving, social and conflict management skills.
